New One-Time Student Symptom Screening Self-Certification Agreement

Dear Dimmick Families, I want to make you aware of a few changes to operations under the District Pandemic Learning Plan. If a student has been determined to be a "close contact" of a confirmed case, the quarantine period may be reduced from 14 days to 10 days as long as the child does not exhibit any symptoms throughout the 10 day period and the parent requests and receives a notice of release from quarantine from the health department. We have expanded equipment use for the students during recess and physical education classes to provide them with more opportunities to get exercise while at school. Parents can now sign a ONE-TIME "Student Symptom Screening/Self-Certification Agreement" that will replace the daily pink self-certification forms required to board school buses each day as well as the daily symptom and temperature checks upon arrival. The Illinois Department of Health has also updated guidance on the symptoms for exclusions from school. The updated list of symptoms are indicated below and are also found in the agreement form. Fever of 100.4 F of higher New onset of moderate to severe headache Shortness of breath New cough Sore throat Vomiting Diarrhea Loss of sense of taste or smell Fatigue from an unknown cause Muscle or body aches from an unknown cause Please complete the following form by Monday, March 22.
